Please complete each page and bring them with you to your scheduled appointment along with your photo ID and insurance card.
If you are unable to complete the New Patient Forms prior to your appointment, please arrive at least 30 minutes before your scheduled appointment time to complete the paperwork.
After your first visit, we encourage you to request access to our Patient Portal, where you can communicate securely and conveniently with your provider. In the portal you can also keep track of your medical visits, test results, billing, prescriptions and insurance.

FAQs for Patients
Do the providers see patients in person or online?
We see patients both in person and by telehealth, however the initial visit is always in person.
Do you accept Medicare and Medicaid?
Currently, we accept traditional Medicare and some Advantage plans.
Do you accept patients without insurance?
Yes, we accept cash pay.
What is the difference between TMS and Deep TMS?
There are currently two kinds of Transcranial Magnetic Stimulation (TMS) devices available for therapy: the earlier, standard rTMS and the more advanced Deep TMS. In recent studies, response rates were found to be significantly higher for Deep TMS versus standard rTMS.
Is Deep TMS for OCD covered by insurance?
Currently, OCD alone is not covered. However, if a patient presents with Major Depressive Disorder (MDD) and OCD, these two conditions can be treated simultaneously and the cost is mostly covered by insurance with an additional charge to the patient for use of the OCD device.
How do I get access to the Patient Portal
After your first visit, you can request access from our front office staff. You will receive a secure email with detailed instructions for how to log in and set up your account.
